How to Reach Our Clinic

We offer several ways for new and established patients to contact our team. To protect your privacy and ensure efficient scheduling, please follow the guidelines in the drop-down menu below.

    If you are a new patient, please call our office and leave a voicemail with your full name, phone number, and the best time to return your call.

    Our staff will reach out to:

    • Confirm your information

    • Begin the registration process

    • Send your personalized invitation link to our Patient Portal

    Note: Portal accounts cannot be created without an invitation from our office.

  • If your primary care provider or another physician is referring you:

    • They may e-fax your referral and relevant medical records directly to our EHR.

    • This is the most efficient way to begin scheduling, especially for electrodiagnostic studies (EMG/NCS), spasticity management, and specialty physiatry evaluations.

    Once your referral is received and reviewed, our office will contact you to schedule and activate your patient portal.

    You may email us to request a call back. For privacy reasons, please include only:

    • Your name

    • Your phone number

    Please do not include medical history, diagnoses, medications, or other protected health information (PHI) in email, as many personal email servers are not encrypted.

  • The Wayfinder Patient Portal provides:

    • Secure messaging

    • Appointment reminders

    • Intake documents and questionnaires

    • Access to visit summaries and care plans

    Portal access is established only after:

    1. You contact our office, and

    2. We send you a unique registration invitation.

    3. Once you have received a registration invitation go to www.yourhealthfile.com

    4. Log in using your username and password. 

    5. Once logged in, select Review medical record

    6. From here select the menu bar (three lines in top left hand corner) 

    7. Review your personal and health information and make any updates or changes by selecting add allergy, add medication, add medical history, etc. 

    8. Click Save to ensure your updates are recorded. 

    If you have trouble logging in or updating your information, please contact our office for assistance. 

What to Expect After You Contact Us

  1. You leave us a message or request a referral.

  2. Our staff reviews your information and contacts you.

  3. You receive a portal invitation link with instructions.

  4. We schedule your appointment once registration is complete.
